FREE GO FISH & GO ARCHERY PROGRAMS

PERMISSION TO ATTEND: As the minor child’s natural guardian, you must read and acknowledge your consent for your minor child to attend Sonfishers FREE PROGRAMS including The Good Angler’s Club. Membership Duration: Life Time.

PERMISSION TO USE PHOTOGRAPHY: As the minor child’s natural guardian you grant to Sonfishers CCMI, its representatives and associates the right to take photographs of yourself, your child, and your property in connection with the above-identified event(s). I authorize Sonfishers CCMI, its assignees, and transferees to copyright, use, and publish the same in print and/or electronically. I agree that the Sonfishers CCMI may use such photography of yourself and/or your child with or without yourname for lawful purposes, including, for example, such purposes as publicity, illustration, advertising.

WAIVER AND RELEASE OF LIABILITY: As the minor child’s natural guardian, you acknowledge that fishing has inherent risk. You (parent/natural guardian) waives and releases any and all claims against, Calvary Chapel Merritt Island, Sonfishers, or any of the sponsors or participants (“Releasees”) for any claim, cause of action or liability arising out of or related to any loss, damage or injury, including death, while participating in the fishing trip, or in route to or from the event, whether caused by the negligence of the Releasees or any other party. The parent/natural guardian agrees to be prepared for inclimate weather and dealing with fish, wildlife, seasickness and any other matters not otherwise mentioned.

CHALLENGE BY CHOICE: The parent/guardian and minor child (participants)_ agree to be safe and have fun by helping everyone have a good day learning to become and practice Good Angler Skills.
